Presentation Description: One key to an effective safety program is incident and near miss reporting. But what exactly is a near miss? And how can we explain the importance of reporting small incidents to employees? Overcoming barriers to reporting, making the reporting process work for employees, and creating a system to act on and report out on action items are three keys to making a reporting program effective. A case study from a mid-sized water agency will be presented, focusing on their recent implementation of a comprehensive, and simple, safety event reporting system. This streamlined system has increased the number of events reported, reduced time to correct incidents, and increased communication across their workforce.
